Does Medicaid Cover Home Care and Assisted Living?

Medicaid covers both home care and nursing home care, but assisted living coverage varies significantly by state through HCBS waivers.

Medicaid and Home Care

Medicaid covers home care through Home and Community-Based Services (HCBS) waivers in all 50 states. These waivers pay for personal care aides, homemaker services, adult day care, and sometimes home modifications. Eligibility requires meeting both financial criteria (income and asset limits) and functional criteria (needing a nursing home level of care).

Medicaid and Assisted Living

Medicaid coverage for assisted living is more limited and varies by state. About 45 states have HCBS waivers that can pay for assisted living services, but the facility must be Medicaid-certified and there are often waiting lists. Medicaid typically covers the care component but not the room and board portion of assisted living costs.

How to Apply

Contact your state's Medicaid office or Area Agency on Aging to apply for HCBS waivers. An elder law attorney can help with Medicaid planning to protect assets while qualifying for benefits. The application process typically takes 45–90 days.
